What to include on a Medical Receipt
Every document should cover the basics on the left. The items on the right are specific to this type.

- The payer's name

- Amount paid, currency and any tax included

Tips for your Medical Receipt
- Patients filing out-of-network insurance claims or HSA/FSA reimbursements usually need an itemized receipt with dates and service codes — in the US this is often called a superbill.
- Keep personal health information on the receipt to the minimum needed, and share it securely; health privacy laws (such as HIPAA in the US or GDPR in Europe) may apply.
- Show copays, insurance payments and patient payments separately so the patient understands what they owe.
- Use the same patient name spelling as on their insurance card to avoid rejected claims.
